I've been having this problem recently when shooting tethered from a D700 to Capture One (version 5.something) and trying to figure out why. The first few images come in no problem, but then this happens.
Could it be the tether cable? I'm hoping so...
Any insight would be great...

Well I have only seen something close to that once before, mine was split horizontaly and not vertically but same idea. I was not tethered though, but it ended up my memory card was bad (the only time it happened is when I was shooting in continuous High, FPS). It worked fine when I only shot one picture at a time and had a 2 min or more break between shots. I suspect it was the card starting to fail but don't know for sure (I tossed it once I saw the problem).
In your case I might look at the cable and maybe the hard drive on the computer your using (this is where I would start but honestly have no clue). Hope you find the problem.
how long is your usb cord?
trouble shoot this;
try another camera
try another cable
try another computer
Do you have an internet connection / network connection switched on ==> Switch off
Do you have a Virus scanner switched on ==> switch off
Do you have programs that are allowed to look for updates ( or even your operating system itself is looking for updates) ==> Swith off or schedule to midnight..
Cause this looks to me like your computer was handling an interupt during the file transfer from your camera to the computer..
Was you computer doing some other heavy task besides transferring images ?
I agree with dutch nikon that you should disable your firewall or anti virus while you use pc/laptop as direct storage of images from camera .
Hmmm, thanks gentlemen. My tether cable is 15 ft which is the maximum length before you need a signal booster. I think I'll start with the cable and go from there. I'm praying it's not a computer hardware related issue. I've had no signs of any potential problems....it's a new MBP 8GB Ram, 2.66MHz.
I'll also try keeping the wifi off and see how it responds.

I don't know how the tethering work in capture one (I'm aperture user, where You don't need card inside Your camera, and I assume it works the same in capture 1) but I saw similar problems when my CF went wild. Anyway, check the cable - 15ft is a lot for high speed data transfer, especially when You use cheap cables.
So, I tested other cables and it looks like my cable was faulty. Lesson learned...cheap cables aren't worth it.
