Sometime at a weddings My D4 shoot a frame when Im pressing the ISO button in the lower part of the LCD and at the same time moving the speed dial to change the ISO.

I have sometimes bumped the portrait release button while adjusting things....and it results in the same effect. One thinks the adjusting is firing the exposure when it is really bumping the release.
